Astellas Access ProgramSM for Prograf

If you have a patient that needs help accessing Prograf, starting the process is easy. Just follow these steps to find out if your patient qualifies for the Astellas Access ProgramSM to receive Prograf at no cost.

1.

Log-on to Astellas eService.

  • If you do not have an account, it is easy to set up. If you need technical assistance to set up an account or for help with your existing eService account, please call AAS at 1-800-477-6472.
2.

Select "Initiate Access Application" and provide the required information.

  • Requests must be submitted by the prescribing physician or by an authorized facility employee.
3. Retain copies of patient's proof of income.
4. If the patient meets the eligibility criteria, they will be enrolled into the program. Both the patient and the healthcare provider will receive notification of enrollment.
5. Patients that are enrolled in the Astellas Access ProgramSM will receive their Prograf shipment within 2-5 business days of approval notification.

 

We strongly encourage healthcare providers to work with their patients to enroll in the Astellas Access ProgramSM on a proactive and timely basis. However, we understand that occasionally patients contact their healthcare provider with an immediate need to access Prograf due to insurance changes or inability to afford their co-payments. Please call AAS at 1-800-477-6472 for assistance.

If a patient needs immediate assistance with Prograf and has notified the healthcare provider, the Astellas Access ProgramSM may be able to offer assistance via Quick Start™, available under the Astellas Access ProgramSM.
